DA-CMT Form 12 This form should be used by all non-MOD delegates to apply for Acquisition courses. Appropriate fees may depend on any arrangements your organisation has with Defence Academy - Bristol. Please see the individual course descriptions for details of the applicable charging rate.

Certificate of Competence in Purchasing and Supply Examination Booking Form This form should be used by all MOD delegates to apply for Certificate of Competence in Purchasing and Supply Examination.

LVP Manager Data Activation Form Completion of this form is a pre-requisite for the Low Value Purchasing - Managers Course. It is essential that the LVP Manager Data Activation form be completed by the applicant's mentor and attached to the course application form (Acquisition and Business Form 11). |
